Child Tax Identifier

Attention Plan Participants who have children enrolled in
Local 399's Health & Welfare Plan:

The Affordable Care Act (ACA) requires everyone to have health insurance coverage or pay a penalty

via their income tax return.  The Internal Revenue Service has determined that a Social Security Number (SSN)

is the best personal identifier to confirm that this health insurance requirement is being met.

The Fund Office is required by law to request this information from you.  If you believe the Fund Office is missing

information from any of your children covered by the Plan, please call the H&W Office, or download the

form below and return it to the Health & Welfare Department.
